Duties
The outpatient therapist is responsible for providing a variety of services to patients in the outpatient setting. This includes treatment planning, evaluation and management, discharge planning, case management and referrals/requests from other providers as needed.
Detailed job duties include:
- Provide a comprehensive range of services to patients, including assessment and treatment and evaluation/rehabilitation.
- Develop an appropriate plan of care for each patient in accordance with the needs of the client or family member.
- Perform assessments on all clients’ conditions as well as providing information regarding their progress toward goals and objectives.
- Provide therapy interventions that are consistent with established standards of practice.
Qualification
- Licensed as an outpatient therapist in the state of California
- Current CPR certification and/or ability to obtain within 90 days from date of hire or transfer
- Must have experience working with patients .
- Ability to work independently without supervision
- Must possess excellent communication skills, both written and verbal
- Ability to maintain confidentiality regarding patient information
